PLEASE NOTE: Pets must have up-to-date rabies vaccinations to be seen at our clinic. If your pet is overdue, we will administer the rabies vaccine during the appointment. 

For EXISTING Patients

For NEW Patients – Please complete each step as noted below

1. If the patient has been seen by another vet before, please contact them to obtain copies of medical records including vaccine history and any labs done. Email them to us at , or upload them while completing our New Patient Form in the next step…
